Job Description:
TriStar Centennial Medical Center is seeking a dedicated and
skilled Case Manager RN to join our dynamic healthcare team in
Nashville, Tennessee. As a Case Manager RN, you will be essential
in coordinating comprehensive care plans that meet the health and
wellness needs of our patients. You will work closely with other
healthcare professionals to ensure that each patient receives the
highest quality of care, tailored to their specific conditions.
This role requires excellent communication skills, a deep
understanding of patient care coordination, and the ability to
function effectively in a fast-paced environment. Assess patient
needs and develop comprehensive care plans in collaboration with
other healthcare professionals Monitor patient progress and adjust
care plans as necessary Advocate for patients' needs and act as a
liaison between patients, families, and healthcare providers Ensure
compliance with healthcare regulations and maintain accurate
patient records Provide health education and resources to patients
and their families Strong clinical skills in a healthcare setting
Excellent communication and interpersonal skills Ability to
multitask and manage time effectively Experience with electronic
health record (EHR) systems Knowledge of healthcare regulations and
accreditation standards Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Valid Registered Nurse (RN) license
Certification in Case Management (e.g., CCM or ACM-RN)
